Making a referral to Work Medical Direct is a simple process of completing the following steps:

1. Download and print referral pack.
2. Complete the forms.
3. Fax papers to the number advised in the referral pack OR post to Work Medical Direct.
4. Work Medical Direct will contact you by fax, email or post with an appointment date time and location.
5. The report will be dispatched by first class post within 48 hours of the examination.
